Physical Therapy

Physical therapists strive to help you reach your goals and get you back to doing what you love. We examine, evaluate and treat patients and residents to help relieve pain, promote healing and restore function and movement.

People have different body types, movement patterns, therapy needs and treatment responses. Our therapists perform a personalized evaluation, establish patient-specific goals and adjust therapy treatment to generate optimal results for each individual.

Occupational Therapy

Our Occupational Therapists teach patients how to safely and effectively perform activities of daily living, including cooking, bathing and dressing. Through the use of assessment and treatment, our therapists enhance each patient’s ability to develop, recover or maintain their independence with daily tasks.

Speech Therapy

Our Speech-Language Pathologists are experts in the assessment and rehabilitation of communication and swallowing disorders (dysphagia). These disorders may be caused by neurological conditions, such as stroke, Parkinson’s disease and dementia. Specialized therapeutic programs, including vital stimulation and LSVT, focus on improving the swallowing safety and functional communication through individualized strategies, based on the patient’s specific impairments.

Transitional Care

At Diversicare, discharge planning begins at admission. You will meet with our interdisciplinary care team to set realistic discharge goals and design a personal rehabilitation plan.

Our top priority is a safe and seamless transition home. Coordination of community based services, obtaining follow- up with primary care physicians, and providing education and training to caregivers can all contribute to a successful outcome.

Wound Care

Diversicare offers specialized treatment for our patients and residents recovering from a variety of conditions including:

  • Diabetic/neuropathic wounds
  • Venous insufficiency ulcers
  • Arterial insufficiency ulcers
  • Pressure ulcers
  • Post-surgical wounds, grafts and flaps
  • Infected and/or draining wounds requiring IV antibiotics
  • Wounds requiring frequent dressing changes
  • Amputations
  • Post-traumatic wounds

Outpatient Therapy

Our therapy services extend to our communities. Whether you have had surgery, are trying to prevent surgery, or are experiencing orthopedic aches and pains, Diveriscare’s outpatient therapy program can help you achieve your recovery goals.

Cardiac Care

Patients and residents with heart disease often need additional care after a hospital stay to assist in a successful return home.

Patients with the following cardiac conditions may benefit from cardiac care:

  • Recovery from a heart attack
  • Angioplasty or stent placement
  • Recovery from heart surgery
  • Recovery from pacemaker placement
  • Angina (chest pain)
  • Congestive heart failure
  • Beginning stages or high risk of heart disease
  • Peripheral arterial disease
  • Hypertension

Post-Surgical Care

Several factors can affect an individual’s ability to recover following surgery, including age, medical stability and other pre-existing medical conditions. Many, if not most, patients will require some care and rehabilitation following surgery. Diversicare offers comprehensive nursing and therapy for most conditions, with the added benefit of adapting our programs to the tolerance of each individual.

Post-surgical care and rehabilitation may be a benefit for patients/residents undergoing:

Stroke and Neurological Disorders

At Diversicare, we pride ourselves on achieving optimal outcomes with patients and residents recovering from a stroke. Our stroke rehabilitation team includes physical therapists, occupational therapists and speech pathologists who are trained in providing treatments specific to stroke recovery. Our rehabilitation teams understand the medical complexities of stroke and other neurological disorders and use this knowledge to develop comprehensive rehabilitation programs.

Orthopaedic Conditions

Orthopedic conditions involve injuries to the bones or joints, including:

  • Fractures
  • Muscle tears or lacerations
  • Joint repair or replacement
  • Amputations

An orthopedic injury will often require surgical intervention, with a need for post- surgical rehabilitation. Even when surgery is not required, rehabilitation can be a benefit in the recovery process. Diversicare provides rehabilitation for all the above conditions. Pulmonary and Respiratory Care
